Please read the following note from Dr. Liss:
Dear Sir or Madam,
Thank you for writing to us. There are four different ways to have your concerns addressed. They will be listed individually:
1) If you are 65 years old or older, you can stop by our office and obtain your Memory Number, for free, any weekday, 9:00-5:00. No appointment is necessary.
2) If you are between the ages of 55-75, you can stop by our office to have an oral swab for possible entry into research trials, designed to stop Alzheimer’s disease from ever starting. This is also free and can be done without an appointment any weekday, 9:00-5:00.
3) If you 55 years old or older and would like to ask Dr. Liss questions about memory, memory loss, memory research, or Alzheimer’s disease, please join us at our next “Ask the Memory Doctor” Town Hall. For a free reservation and program specifics, please log back onto ColumbusMemoryCenter.Com and obtain your free ticket(s). At this event, you can also obtain your Memory Number and oral swab for free, based on your age, as outlined above.
4) If you would like a formal neurologic evaluation to assess your memory and cognition, a new patient appointment can be obtained. Our office telephone number is: 706-327-4000. As opposed to the other options, this will establish a standard doctor-patient relationship and be governed under normal insurance guidelines. You will either be seen by Dr. Jonathan Liss or Nurse Practitioner Crystal Claridy.
